Complex Palliative Care Plans- Community based
These care plans have been developed in partnership with key local organisations to support community healthcare professionals in recognising and managing common palliative care emergencies. They provide evidence-based guidance for patients with complex symptoms who wish to remain in the community and avoid hospital-based treatment. The current plans cover end-of-life bleeding, seizures, diabetic complications, and metastatic spinal cord compression, with further details available following this link.
