These care plans have been developed collaboratively with St Barnabas Hospice, Lincolnshire Community Health Services, NHS East Midlands Ambulance service and the United Lincolnshire Teaching Hospitals Trust to support community healthcare professionals in recognising and responding to common palliative care emergencies.

The aim was to develop a resource that reflects the best available evidence with complex symptoms who wish to remain in the community and do not want escalation to secondary care for active treatment.

Currently we have care plans for those at risk of an end-of-life bleed, seizures, diabetic complications and metastatic spinal cord compression. Please see below for further information.

Please remember that these care plans should only be put in place when people are in the final stages of life and their focus of care is to stay at home or in their usual place of residence (residential or nursing home) and whose wish is to not be admitted to hospital or it is not in their best interest. Always update a patient’s ReSPECT form to reflect their wishes.
