Swan Bags

The SWAN homecare bag initiative has been introduced to help family and carers to support a person in the last days of life. The bag contains several useful items to provide a few days of supplies until professionals have been able to assess and order items needed for ongoing care. They can be handed out on discharge from hospital or support patient in the community as appropriate.
