SPICT - 4 ALL
Caregivers and family members can often see that a person is getting less well with one or more health problems. Making plans for care now and in the future is important.
It can be difficult to know when to ask for more help and support or how to talk about this.
SPICT-4ALL uses non-medical words but is similar to the SPICT for health professionals. You can use it to help you ask about more help from a doctor, a nurse or another professional.
You may be the person who knows what is important, what is needed and what could help.
What’s important to me: message in a bottle.
Lions Message in a bottle – Internationally recognised. There are stickers to identify someone has a bottle in the fridge supplied with each bottle.
It is the PERSONS document – not professionals.
• Although there is guidance on the form the person can add, draw, highlight attach whatever they want people to know in an emergency.
• It needs to be completed in English so the person reading it in an emergency can understand “what’s important”.
The neighbourhood Teams can support those who can’t complete the form themselves.
Welfare and benefits advice
Living with a life-limiting or terminal illness, or caring for someone who is, can lead to serious financial problems. St Barnabas Welfare Services Team can help you understand what financial help is available.
They can provide practical support with:
- Finding out what you’re entitled to
- Applying for benefits and grants (e.g. completing forms, making phone calls or writing letters)
- Accessing help with health costs, such as travel to hospital
- Applying for transport concessions, such as a disabled parking badge
- Advising on benefits decisions
- Accessing benefits following a bereavement and money towards funeral costs

It’s a good idea to speak to the Welfare Services Team, who will be able to help you understand your options. A member of the team can help you to claim benefits and other income you are entitled to. This can be done either by telephone, a home visit or at one of the Day Hospices.
If appropriate, a member of the team can make the claim for benefits on behalf of the individual. This includes completing the paperwork.
