SWAN Homecare Bags

Earlier this year we introduced a SWAN Homecare Bag scheme, which is available to be accessed for Palliative and End of Life patients.

The attached information provides the details of the scheme, including visual poster with bag content, comms for sharing – including where to access the bags from, as well as the information sheet added to each bag for patient and their family. The SWAN bags contain a number of items to help family and carers to support a person in the last days of life. They are designed to provide a few days of supplies until professionals have been able to assess and order items needed for ongoing care.

They can be handed out on discharge from hospital or support patient in the community as appropriate.
