Fast-track care packages:
• All referrals and increases to be emailed to licb.ft@nhs.net.
• New referrals are made using the attached SPA referral form.
• Packages for care at home for individuals agreed as fast-track eligible are managed by the All-age Continuing Care team.
• Any communication regarding a fast-track package of care should be sent to licb.ft@nhs.net.
• Marie Curie nights are managed through the All-age Continuing Care team.
• The All-age Continuing Care team are working 9am – 5pm, 7 days a week to ensure all referrals are processed in a timely manner.
Planned palliative care:
• See the attached SPA referral form - the form must be sent to lhnt.lchsreferrals@nhs.net.
• The LCHS Operations Centre process and forward the referral onto the relevant team as marked on the referral form by the referrer.
• This is completed 7 days a week between 9am-5pm.

A tool used by clinical professionals that attempts to score a patient's needs and the appropriate response. Developed by a partnership of providers in Lincolnshire: ULHT; Macmillan; LCHS; Marie Curie; St Barnabas Hospice; LCC.
