Important! Please note when referring to any hospice- if you are referring a patient who required 1-1 support, please liaise with the hospice directly to discuss as acceptance may depend on staffing levels. Failure to do so may negatively impact patient safety.

St Barnabas Hospice in the Hospital (Grantham)
Hospice in the Hospital referral process:
Referrals will be accepted from any health or social care professional. Patients must be registered either on a permanent or temporary basis with a Lincolnshire GP. If the patient does not have a Lincolnshire GP, the team will need to seek permission for admission from ICB and in some circumstances the team will seek support from the Lead GP to guide admission to the unit.
Referrals can be made 24 hours a day. The referrer will be required to complete an admission pack. Support to complete this can be offered by contacting the nurse in charge via telephone on 01476 464988.
Butterfly Hospice (Boston)
Referrals will be accepted from any health or social care professionals only. Patients must be registered either on a permanent or temporary basis with a Lincolnshire GP. If the patient does not have a Lincolnshire GP, the team will need to seek permission for admission from ICB and in some circumstances the team will seek support from the Lead GP to guide admission to the unit. Referrals can be made 24 hours a day.

If a patient is being admitted to the hospice for symptom management advice but they are not registered with a GP within the North Lincolnshire area further discussion in relation to funding may be required prior to referral- please call: 07966137422 for further details. If a patient is being admitted for end of life care discussion around funding may not be required.
