Referral Criteria: Patients must be over 18 years old and deemed ‘medically fit’ or do not wish for escalation/ treatment (this must be documented on their ReSPECT form). Patients must also be registered with a Lincolnshire GP.
How to refer: Complete a Home First Form (see details below) and email it to LHNT.homefirst@nhs.net. To check bed availability please call 01522 459 790. Please note that they only accept health professional referrals.
Referral criteria: Patients must be over 18 years old. GP beds are short term 4-6 week beds and are for crisis admission for End of Life Care or symptom management. We are unable to accept mobile patient with Dementia/confusion as we are not a secure unit. GP beds are not just for Palliative care and can be used by the Community Teams for health-related issues, such as wound care, self-neglect, reduced mobility due to acute episode of illness, hospital avoidance.
How to refer: For GP beds contact for bed availability and referral by calling 01406 422283. Fast Track will need to be completed for admission into the Nursing Home. *Referrals should be made 9am-5pm Monday to Friday, but they can accept referrals in an emergency at any time depending on bed availability.
